Per Federal Law we are implementing Electronic Medical Records (EMR) to comply with guidelines for Meaningful Use.
Meaningful Use is defined by the use of certified EHR (Electronic Health Records) technology in a meaningful manner (for example electronic prescribing); ensuring that the certified EHR technology is connected in a manner that provides for the electronic exchange of health information to the Secretary of Health & Human Services (HHS) information on quality of care and other measures.

All office visits are by appointment only so that every patient receives the individual attention necessary. Please respect the appointment time you reserve and make every effort to give at least 24 hour notice of any unavoidable cancellations.
**Failure to give 24 hour notice of a cancellation will result in a charge as stated by our patient paperwork.**

Fees – Payment is expected at the time of service for all co-payments, deductibles, co-insurance, and non-covered or elective procedures. We accept cash, check, Visa, Master Card, AMEX, and Discover.
